Most cysts are benign — slow-growing lumps beneath the skin that cause little trouble until they become noticeable, uncomfortable, or infected. What many patients call a "sebaceous cyst" is usually an epidermoid or pilar cyst. Epidermoid cysts typically form on the face, neck, back, and chest; pilar cysts are particularly common on the scalp.

Definitive treatment involves removing the cyst and its complete sac under local anaesthetic. Simply draining or squeezing the cyst does not remove the sac — the cyst will often refill and return. Surgical excision, where the sac is removed in full, gives a significantly more lasting result.

The procedure. Local anaesthetic is injected around the site. A small elliptical incision is made; the cyst and sac are carefully removed. Where appropriate, deeper sutures may be used to support the wound and reduce tension on the skin closure. The wound is then closed with sutures and a dressing applied. Tissue may be sent for histological analysis where clinically appropriate.

Most patients return to desk-based work the same day or next, depending on the site. Keep the dressing clean and dry for 48 hours; avoid strenuous activity until sutures are removed — usually 7–14 days after surgery.

Possible complications include wound infection (usually treatable with antibiotics), bleeding or bruising, altered sensation, recurrence if any sac remains, pigmentation change, and scarring. A new cyst may form elsewhere in future. Plastic surgery closure techniques aim to optimise scars, but no incision can guarantee an invisible result.

The most appropriate approach depends on the cyst's size, location, and whether it is currently inflamed or infected. Your exact price is always confirmed before treatment begins.

Observation. For small, symptom-free cysts causing no concern, monitoring for any changes is a reasonable choice.

Incision and drainage. Releases cyst contents to relieve pressure. The sac is not removed, so the cyst will likely return. Not a definitive solution.

Surgical excision. The cyst and its complete sac are removed under local anaesthetic, followed by neat closure. This is the definitive treatment — the approach we most commonly recommend where removal is appropriate.

Antibiotics. If the cyst is infected, antibiotics may be prescribed to treat the infection, although this will not remove the cyst itself. Definitive excision is then planned once inflammation has settled.

12 Will I have a scar after cyst removal?

All surgical incisions leave a scar. We use plastic surgery closure techniques — including deeper sutures where appropriate — to make the scar as fine and inconspicuous as possible. Most scars fade well over several months. No procedure can guarantee an invisible result.

13 Does the removed tissue need to be sent for histology?
